This amendment is issued to provide responses to a vendor inquiry. The Government received an inquiry regarding Pharmacy services for the Yakama Service Unit, located in Toppenish, WA. Acknowledgment Requirement: Respondents must confirm in their quote submission that they have reviewed and will comply with all information,

requirements, and conditions provided in this amendment. Failure to acknowledge this amendment may result in the quote being deemed non-responsive. The following is a list of question(s) received in regards to subject solicitation with responses: Does the solicitation require a candidate to be presented, if so, requesting copies of required form listed within the solicitation. Response: The Government will work with Awardee when the Purchase Order is executed to complete required documents. All other terms and conditions remain unchanged. AMENDMENT TO COMBINED This amendment is issued to revise the solicitation closing date. Because the original closing date falls on a Federal holiday, the closing date is hereby extended to the next business day.
