1 TO COMBINED SYNPOSIS/SOLICITATION This amendment is issued to provide response to interested vendors' inquiries and to revise the Schedule of Items to add a Unit of Measure column. Attached is Attachment 4 -

Questions and Answers and Attachment 1 - Schedule of Items - Medical Supplies - UOM Revised. This is a combined for commercial products prepared in accordance with RFO FAR Part 12. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation No. 75H71326Q00007 has been issued as a Request for Quotation (RFQ) for a firm-fixed-price commercial products contract for Medical Supplies on an "As Needed Basis" for the Indian Health Service, Portland Area Office, Western Oregon Service Unit, Salem, Oregon. The requirement includes medical supplies for the Western Oregon Service Unit, including the items listed in Attachment 1 - Schedule of Items. Supplies shall be furnished on an as-needed basis, delivered within 3 to 10 business days after order, and provided in accordance with Attachment 2 - Specifications Sheet. The anticipated

period of performance includes one 12-month base period and four 12-month option periods. The base period is anticipated to be July 20, 2026 through July 19, 2027. Each option period, if exercised, will continue for 12 months following the prior performance period. The actual start date and end date will be stated in the resulting award. The place of delivery/performance is the Western Oregon Service Unit campus, Chemawa Indian Health Center, 3750 Chemawa Road NE, Salem, Oregon 97305.
