This is a REQUEST FOR INFORMATION and SOURCES SOUGHT for the Endoscopy Reprocessing Sink at the Wyoming Clinic Contractors that deem themselves capable of meeting the requirement shall provide the below information to Percy Johnson,

Point of Contact Name, Phone Number and E-mail Address (4) DUNS and NAICS code (5) Business Size SMALL or LARGE (6) Type of Small Business (if applicable): service disabled veteran owned, veteran owned small business, 8a, HUBZone, woman-owned, etc. (7) A concern that is not a manufacturer of the supply shall provide the manufacturers name and size standard (8) Country of Origin. requirements below: Salient Characteristics/Statement of Need or Work VISN10-515 WYOMING HEALTHCARE CENTER ENDOSCOPY REPROCESSING SINK General: This request is to replace the Endoscopy Reprocessing Sink at the Wyoming Clinic, a CBOC to the Battle Creek VAMC, for the Sterile Processing Service (SPS). The current equipment was original to the building and is noncompliant with VHA Directive 1116(2), and the Association for the Advancement of Medical Instrumentation (AAMI) standards set forth by the Joint Commission, AMMI ST91. This has been deemed a Priority 1 replacement (Mission Critical) for the Gastrointestinal (GI) clinic s reprocessing area within SPS during the recent site visit and audit from the VISN 10 Chief Sterile Processing Officer (CSPO). The C&A

requirements do not apply, and a Security Accreditation Package is not required. Warranty

Requirements: General manufacturer warranty that products are free from defects in materials and workmanship. All defects will be replaced or repaired by the manufacturer at no cost to the buyer. The manufacturer directly offers a three-year warranty on the lifters on the adjustable height sink made by PureSteel Healthcare (requesting specific sink, documentation will be included in FORCE package).
