This is a combined for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) parts 12 and 15, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation document will not be issued. This solicitation is a request for proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition (FAC) FAC 2023-04, dated 06/02/2023. This requirement is solicited as a Service-Disabled Veteran Owned Small Business (SVOSB) set-aside. All offerors must be verified in the Vendor Information Pages, https://www.vip.vetbiz.gov/, and must be registered in System for Award Management (SAM) at https://www.sam.gov to be considered for an award of a federal contract registered in the record must be complete and not missing elements such as representations and certifications. requirements contract for Home Oxygen Home Oxygen services and supplies for beneficiaries of the Bay Pines VA Healthcare System (BPVAHS). Contracted services are to be extended to patients managed in their home environment and may extend to veteran patients in a nine-county service area of the BPVAHS. Contractor shall provide all supplies, materials, equipment, labor, supervision, management, transportation, safety management, infection control, repairs, maintenance, and in-service training for patients to include patient education as defined in the Statement of Work (SOW). The contractor will be responsible for providing, installing, and servicing medical oxygen equipment, high-pressure cylinder, concentrator and/or liquid systems, portable systems, emergency back-up systems and supplies to eligible VA patients as required by the BPVAHS. .All
