BACKGROUND: Strategic Weapons Facility Pacific (SWFPAC) currently has a MAMAVA Lactation Pod to be relocated to Naval Base Kitsap Building 1006. 2. SCOPE: -Disassemble one Mamava MV-8008-OR lactation pod in (Building 6589), transport it to new location (Building 1006), and reassemble Mamava POD in new location (Building 1006). - Building 6589 and 1006 are located at Naval Base Kitsap-Bangor in Silverdale, Washington. See attached site map for further details. - Building 6589 and 1006 have an area that is accessible to drive up directly to the building. The building entry way consists of a double door. There are no stairs. The disassembly and reassembly of the lactation pod will be located on the ground floor where the entry way opens up into. 3.

PERIOD OF PERFORMANCE: Period of one (1) business day for disassembly, transport, and reassembly of the Mamava lactation pod unit. Work shall be performed between the hours of 0700 and 1500. Arrangements for exact date for performance will be made with the Government

point of contact upon contract award with performance commencing within 30 calendar days of contract award. 4. MATERIAL TO BE PROVIDED BY THE GOVERNMENT: None 5. MATERIAL TO BE PROVIDED BY THE CONTRACTOR: The contractor is responsible for providing all tools. equipment, and vehicles necessary for the disassembly, transport, and reassembly of the Mamava lactation pod unit. 6.

PLACE OF PERFORMANCE/LOCATION OF WORK: The services under this contract shall be performed on-site located at SWFPAC's facility on Naval Base Kitsap - Bangor, located in Silverdale, Washington. Building 6589 and 1006 are located within 2.3 miles of each other (see provided map of route). Building 6589 is located on Roosevelt Street, Naval Base Kitsap-Bangor and Building 1006 is located on Skate Street, Naval Base Kitsap-Bangor. 7. QUALITY ASSURANCE

REQUIREMENTS: The Vendor is solely responsible for the quality of the assessment and services to be provided. Services must be performed by a trained technician that has the proper knowledge, instructions, and training to dissemble and reassemble a Mamava model MV-8008-OR lactation pod. 8. TRAVEL
