Description

Provide confidential assessment, counseling and referral services, supervisory training and mental health education for the approximately 1200 employees of the U.S. Army Corps of Engineers, New Orleans District (MVN). This is a combined for a commercial service procurement prepared in accordance with the format in RFO 12.202, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation Number W912P8-26-Q-A020 is being issued as a 100% Small Business Set-Aside only procurement under a Request for Quote (RFQ). It is being procured as Firm-Fixed Price under RFO Part 12, Acquisition of Commercial Items. The designated NAICS code is 624190 – Other Individual and Family Services, Size Standard $16,000,000.00. The PSC code for this requirement is Q999 – Medical - Other. The basis of award will be Lowest Priced Offeror(s). Additionally, Offerors must quote on all items to be eligible for award. The Government will award one (1) contract resulting from this solicitation to the responsible Offeror(s) whose Offer conforms to ALL

requirements of the solicitation. General

Instructions to Offerors: All Offerors MUST be registered in SAM.gov as a Small Business under NAICS code 624190 to be considered for award. NOTE: (RFO 52.204-7) In accordance with RFO 4.203, states prospective Offerors shall be registered in the Department of Defense "System for Award Management" (SAM) database at the time of submission of its offer. If your company is not registered in SAM you may do so by going to: http://www.sam.gov. You may be required to provide your company's unique entity identifier (UEI) issued by the GSA.
